Make a Referral ...

To refer a youth to any Bridge program:

  1. Email us at wildernesscenter@diakon.org to initiate the referral process. You may also wish to contact us directly by telephone as noted:
    • Delaware County: (610) 291-7731
    • Chester County: (610) 334-0355
    • Lancaster County: (717) 572-1128
    • York County: (610) 291-7731
    • Adams, Cumberland, Franklin and Perry counties: (717) 960-6700
    • Other counties’ referrals or private referrals (717) 960-6700 or (610) 291-7731
       
  2. Submit all information pertinent to the referred youth’s background including school records, medical history, psychological testing, diagnostic report, family/social history, and recommendation (if available) for admissions staff review.
